Participant Waiver, Release of Liability & Assumption of Risk Agreement
PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS.
1. Parties
Released Parties: The Wolves Edge Combat Sports, its owners, officers, instructors, employees, volunteers, affiliates, agents, representatives, successors, and assigns (collectively, “Wolves Edge”).

2. Activities Covered
Participation in Brazilian Jiu-Jitsu instruction, sparring, drills, strength & conditioning sessions, open-mat practice, seminars, competitions, the use of on-site equipment and facilities, and attendance or participation in any related events on or off premises (collectively, the “Activities”).
3. Assumption of Inherent Risks
I understand that participation in Brazilian Jiu-Jitsu and related Activities involves inherent risks which cannot be completely eliminated regardless of the care taken to avoid injuries. These risks include, but are not limited to:
• Strains, sprains, fractures, cuts, bruises, joint dislocations, concussions, paralysis, and other serious bodily injuries;
• Collisions with training partners, objects, or equipment;
• Exposure to infectious diseases, including but not limited to MRSA, COVID-19, influenza, and skin infections;
• Aggravation of pre-existing injuries or medical conditions;
• Psychological stress, anxiety, or trauma;
• Risks associated with potential negligent first aid or emergency response.
I VOLUNTARILY AND KNOWINGLY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES.
4. Health & Medical Declaration
I certify that I (or my minor child) am physically and mentally capable of safely participating in the Activities and have consulted a qualified physician if necessary. I agree to promptly inform Wolves Edge of any medical or physical condition that may impact my participation. I authorize Wolves Edge to seek emergency medical care on my behalf if deemed necessary, understanding that I am responsible for all associated medical costs.

5. RELEASE OF LIABILITY & WAIVER OF CLAIMS
I HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE THE RELEASED PARTIES FROM ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, LOSSES, OR LIABILITIES ARISING DIRECTLY OR INDIRECTLY FROM MY PARTICIPATION IN THE ACTIVITIES, INCLUDING CLAIMS ARISING IN WHOLE OR IN PART FROM THE NEGLIGENCE OF ANY RELEASED PARTY, TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW.
6. Indemnification & Parental Authority
I agree to indemnify, defend, and hold harmless the Released Parties from any claims, damages, legal fees, medical expenses, or other costs incurred by me, my family members, estate, heirs, assigns, or any third party due to my involvement in the Activities.
If the Participant is a minor, the undersigned parent or legal guardian represents that they have full legal authority to enroll the Minor in the Activities and to execute this Agreement. The undersigned further agrees to:
• Be jointly and severally responsible for all tuition, fees, and medical costs arising from the Minor’s participation; and
• Indemnify and hold harmless the Released Parties from any claim, demand, or action brought by another individual asserting legal or custodial rights over the Minor Participant.
7. Media Release
I consent to being photographed, recorded, or filmed during my participation in the Activities. I authorize Wolves Edge to use my likeness, name, voice, image, and statements for promotional, marketing, or educational purposes without compensation.
8. Facility Rules & Conduct
I agree to:
- Comply with all posted rules and verbal instructions from Wolves Edge staff.
- Maintain excellent personal hygiene and wear clean training attire at all times.
- Refrain from profanity, abusive language, or inappropriate conduct.
- Inspect the training area and equipment prior to use and promptly report any hazards.
Violation of these rules may result in suspension or termination of membership without refund.
9. Severability & Governing Law
If any provision of this Agreement is found to be invalid or unenforceable, the remainder shall continue in full force and effect. This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, excluding its conflict-of-law rules.
10. Term & Renewal
This Agreement remains in effect from {sign_date} until revoked in writing by the Participant (or Parent/Guardian) and must be renewed annually to remain valid.
11. Acknowledgment of Understanding
I have read and fully understand this Agreement and sign it voluntarily, recognizing that I am waiving substantial legal rights, including my right to sue.
